Hold Harmless Agreement, Camp Waiver, and Release of All Claims

Please read the following carefully. By registering and participating in the 770 Sports Performance Speed Camp, you acknowledge and accept the terms outlined below.

As the parent or legal guardian of the participant, I fully understand and acknowledge that participation in athletic training involves inherent risks, including but not limited to physical injury. I agree to assume full responsibility for any and all injuries, damages, or losses that my child or I may sustain as a result of participation in any activities associated with this program.

I hereby voluntarily waive, release, and discharge any and all claims for damages for personal injury, property damage, or loss that may arise from participation in the 770 Sports Performance Speed Camp. This release applies to all claims resulting from or in connection with participation in the program, whether caused by negligence or otherwise.

I agree to hold harmless and indemnify 770 Sports Performance, its owners, staff, agents, representatives, and volunteers, as well as the Cherokee County Board of Education (CCBOE), Cherokee County School District, and its employees, from any liability, claims, demands, or expenses, including legal fees, arising from my child’s participation or use of the school district's facilities.

Insurance Information

I understand that health, medical, and hospital insurance coverage is the sole responsibility of the participant, parent, or guardian. 770 Sports Performance does not provide accident or health insurance coverage for program participants.

I have read and fully understand this Waiver and Release of Liability. The following selections and my signature below confirms my agreement to these terms.

I have read and agree to the waiver and release of liability. I understand that speed training involves physical activity, and I accept all responsibility for my child's participation.

I authorize 770 Sports Performance to photograph and/or video my child during training sessions and to use these images for promotional purposes, including social media, the website, and marketing materials. No names will be used without additional permission, and no compensation will be provided. If I do not wish for my child’s image to be used, I will notify 770 Sports Performance in writing.

In the event of an accident or medical emergency, I authorize 770 Sports Performance and its representatives to seek appropriate medical treatment and transportation for my child. I understand that every effort will be made to contact me or the listed emergency contact before treatment is provided.
